If you are interested in becoming a supplier or subcontractor for Houchin Construction Inc., please forward the following information via email to billing@houchinconstruction.com

1. Completed Subcontractor Prequalification Form. (click here to download)
2. Completed W-9 form. (available from www.irs.gov)
3. Copy of your State and/or County license.
4. Certificate of Insurance with the following requirements (click here for a sample Certificate of Insurance):
   1. General Liability Insurance of $1,000,000 per occurrence limit, $2,000,000 Products & Completed Operations
   2. General Liability Insurance of $2,000,000 General Aggregate limit
   3. Subcontractors must name the General Contractor, Owner and Architect as Additional Insured, including Completed Operations, on General Liability policy
   4. General Liability policy must be primary/non-contributory
   5. A Waiver of Subrogation in favor of the General Contractor must be included on General Liability and Workers Compensation Policies
